Please follow these steps exactly as they are listed. Deviation from these steps may result in the unsuccessful setup of MFA.

1. Purchase a Hardware token from https://shop.ftsafe.us/products/otp-c200-i34
   
   
2. You will receive an email with the serial number and the 13 digit key printed on the back of the token.
   
   (example back of token)
   
   
   
3. Email ITS@lsus.edu with the subject Hardware Token and in the body include the following information:
   
   Student ID#
   Hardware token serial number
   13 digit key printed on the back of the token
   
   
4. Wait for confirmation from ITS that your token was successfully added to your account.
   
   
5. Login to lsus.edu/mfasetup using the One-Time 6 digit code that appears on your hardware token display to verify your new token was successfully added.
